Stockton Cardiology has a remote care service called Chronic Care Management for eligible patients who need extra help managing their health. The purpose is to prevent you from being hospitalized, by enabling your cardiologist to have more control over your well-being. This program provided by Enable Healthcare in coordination with your cardiologist.
Chronic Care Management helps you stay healthy from the comfort of your home.
Letter to Identified Patients
If you have received a letter from Stockton Cardiology stating that you have been identified as eligible to receive this program, but want to know more, call the nurses at Enable Healthcare at 209-299-1679 for enrollment information.

What is the CMS Chronic Care Management program?
The program is designed to help you maintain a healthy lifestyle, stay on top of refills and appointments, and address general questions and concerns monthly. The goal of the program is to prevent you from being hospitalized by enabling your provider to have more control over your well-being. We strive to improve patients’ quality of life.
Who is the program for?
The CCM program is for anyone with two or more chronic conditions. These chronic conditions include, but are not limited to, hypertension, diabetes, arthritis, COPD and dementia. Best of all, this program is covered by your insurance!
How Chronic Care Management Works
- Identify Your Eligibility – Based on your medical history we will verify if you’re eligible for the CCM program.
- Getting You Enrolled – Our CCM nurses and your cardiologist’s office will give you info on how you can start the CCM program.
- Review Care Plan – Your cardiologist will give you a personalized care plan based on your medical history, which is shared with our nurses.
- Monthly Nurse Calls – Our CCM Nurses will reach out to you at least once a month to perform health checks.
- 24/7 Nurse Access – You have the ability to call our CCM Nurse Hotline to address any of your healthcare needs.
