Cardiac rehabilitation can help many individuals achieve their recovery goals and avoid a repeat visit back to the hospital. Home-based cardiac rehabilitation has been shown to be safe and effective, allowing you to participate from the comfort of your own home when it works best for your schedule.
Throughout the program, you’ll receive educational materials, support group resources, and counseling regarding heart disease, diabetes, medications, nutrition and weight loss, stress management, mental health, risk factor identification, modification, and smoking cessation.
This comprehensive approach ensures that you’ll receive the knowledge, support, and tools you need to improve your heart health and overall well-being.

How it works

- When you enroll in the program, you will be sent a Cardiac Recovery Kit with all you need to participate in your sessions.

- Many patients join sessions using their tablet, smartphone, or computer with a web camera. If you don’t have access to a device, we will send you a loaner internet-enabled tablet so you can see your cardiac therapist face-to-face as they guide you step-by-step through your sessions.

- Once you receive your Cardiac Recovery Kit, you will meet with one of Recora’s orientation exercise physiologists to discuss your health goals and clinical history. They will create a personalized treatment plan for you and will tailor it to your needs throughout your time in the program.

- Based on your treatment plan and schedule, you will be matched with a cardiac therapist. You will meet with your care team two to three times a week for approximately 12-18 weeks. They will also keep in contact with your Stockton Cardiology care team to provide updates. You will be encouraged to participate in both individual and group sessions to provide the maximum health benefit.

Who is Remote Cardiac Rehabilitation (RCR) available to?
Your doctor will determine if this program is recommended for you. Cardiac rehabilitation is sometimes indicated following a heart attack, heart procedures (such as stent placement, coronary artery bypass surgery, or valvular surgery) as well as for patients who chronically manage heart failure or stable angina.

Care Insider provides support to further improve access with your care team at Stockton Cardiology Medical Group. You can contact health coaches as well as have increased communication options with Stockton Cardiology through a technology platform and direct phone line.

This program is more comprehensive than Remote Cardiac Rehabilitation. The goal is to achieve and maintain risk reduction by making measurable improvements to the conditions affecting your heart disease.
A few of the benefits include: health reminders, enhanced scheduling options, and special access to group exercise, nutrition, and wellness classes.
Care Insider, an Advanced Primary Care Management Program, provides you with:
- Personalized care coaches to discuss your health needs
- Access to live nutrition, exercise, and wellness classes
- Screenings for cardiovascular and wellness issues
- Assistance after hospitalizations or transitions in care
- Virtual cardiac rehabilitation screening program
- Telehealth assistance and technology support
- Support finding financial assistance programs, transportation, food, housing, or other health-related services
How it works
- Enroll by providing your doctor or Recora with your consent to join the program.
- Upon enrollment, a member of Recora’s care team will reach out to discuss your health needs, develop your personalized care plan, and answer any questions you might have using the Care Insider resources
- While enrolled, you have access to all of the above services as well as phone or virtual check-ins to discuss your care
